Abstract
Neuropathic pain is initiated or caused by primary lesion or dysfunction in the peripheral or central nervous system that almost always a chronic condition. There are many kinds of peripheral neuropathic pain such as diabetic peripheral neuropathy, post herpetic, post surgical, trigeminal neuralgia etc. Peripheral diabetic neuropathy is one of chronic complications of diabetes mellitus based on metabolic stress and then microvascular abnormalities. These disturbances is believed arise from a lacking regulation of blood glucose that provokes the acceleration of degenerative process. Blood glucose regulation by using diet, exercise and may be pharmacolocical therapy will be the main priority in the treatment of peripheral diabetic neuropathy. The goal of treatment of neuropathic pain will be to alleviate pain, treat comorbid conditions and improve function of the organ in order to get better quality of life.. However, irreversible degenerative processes make neuropathic pain responds poorly to conventional analgesics. The current concept of treatment of neuropathic pain is in modulation of hyperexcited neuron with pregabalin ( Lyrica® ) in order to suppress neurotransmitter production. Pregabalin is proven as a safe and fast acting drug for neuropathic pain with additional benefits on improvement of sleeping disorder and anxiety. Keywords.Diabetes mellitus, metabolic stress, peripheral neuropathic pain, hyperexcited neuron, pregabalin
