Abstract

Type 2 diabetes mellitus ( T2DM ) is a progressive disease characterized by declining pancreatic β cell function leading to decreased plasma insulin levels, and insulin resistance. A maladaptive response to insulin resistance may lead to a vicious cycle with more impaired insulin secretion and progression from normal glucose tolerance ( NGT ) to IGT ( prediabetic state ), and T2DM and induces transient endothelial dysfunction. This process is triggered and accelerated by environmental factors. Excess expossure to hyperglycemia ( glucotoxicity ) may lead to tissue damages such as cardiovascular complications. Management of hyperglycemia, the hallmark of type 2 diabetes mellitus ( T2DM ), has historically taken center stage in the treatment of this disease. Maintaining glycaemic levels as close to the normal range as possible has been demonstrated to have a powerful beneficial impact on tissue damages. Improving insulin resistances and insulin deficiencies will be the best rational treatment in order to get optimal effect on cardiovascular protection More than one medication is usually necessary for the majority of diabetic patients overtime. In general, antihyperglycaemic drugs with different mechanism of action were needed to have the greatest synergy. It is reasonable to consider antidislipidemic and anti hypertensive agent in conjunction to commonly presence of metabolic syndrome in T2DM. Glimepiride as a third generation sulfonylureas provides the double action such as insulin sensitizer and insulin secretagogue to improve hyperglycemia. Statin and ACE inhibitor or Angiotensin Receptor Blocker are recommended to dislipidemia and hypertension in T2DM.
